On the origin and continuing evolution of SARS-CoV-2

ABSTRACT
The SARS-CoV-2 epidemic started in late December 2019 in Wuhan, China, and has since impacted a large portion of China and raised major global concern. Herein, we investigated the extent of molecular divergence between SARS-CoV-2 and other related coronaviruses. Although we found only 4% variability in genomic nucleotides between SARS-CoV-2 and a bat SARS-related coronavirus (SARSr-CoV; RaTG13), the difference at neutral sites was 17%, suggesting the divergence between the two viruses is much larger than previously estimated. Our results suggest that the development of new variations in functional sites in the receptor-binding domain (RBD) of the spike seen in SARS-CoV-2 and viruses from pangolin SARSr-CoVs are likely caused by mutations and natural selection besides recombination. Population genetic analyses of 103 SARS-CoV-2 genomes indicated that these viruses evolved into two major types (designated L and S), that are well defined by two different SNPs that show nearly complete linkage across the viral strains sequenced to date......

The novel coronavirus belongs to the coronavirus, and its genetic characteristics are obviously different from those of SARSr —Cov and MERSr —CoV. The virus is sensitive to UV and heat. The virus can be inactivated by 56 °C for 30 minutes, ether, 75% ethanol, chlorine containing disinfectant, peracetic acid and chloroform.

WHO director-general Tedros Adhanom Ghebreyesus, who made a trip to Beijing for talks with President Xi Jinping and Chinese ministers in late January, returned with an agreement on sending an international mission.

But it has taken nearly two weeks to get the government’s green light on its composition, which was not announced, other than to say that WHO veteran Dr. Bruce Aylward, a Canadian epidemiologist and emergencies expert, was heading it.

“I’ve just been at the airport seeing off members of an advance team for the @WHO-led #2019nCoV international expert mission to #China, led by Dr Bruce Aylward, veteran of past public health emergencies,” Tedros said in a tweet from Geneva.

Dr. Sylvie Briand, who accompanied Tedros last month and stayed behind for talks with top Chinese health officials, told Reuters last week that they were discussing a list of experts with China.

“Because it is a joint mission, they need to be on board, it’s not just an international group going there. We have about 15 people,” said Briand, director of Global Infectious Hazard Preparedness at WHO.

China raised the death toll from the coronavirus outbreak to 811 on Sunday, passing the number killed globally by the SARS epidemic, as authorities made plans for millions of people returning to work after an extended Lunar New Year break. [L4N2A9045]

The virus, which has spread to two dozen countries, has killed some 2% of more than 37,550 cases worldwide, with 99 percent of infections in China, WHO figures show.

The WHO declared the outbreak a global emergency on Jan. 30, days after the Chinese central government imposed a lockdown on 60 million people in Hubei province and its capital Wuhan, epicenter of the virus that emerged in December in a seafood market.

Tedros said on Saturday that he hoped the team would include experts from the U.S. Centers for Disease Control (CDC).

DISPOSAL of PPE - Personal Protection Equipment & Other Info See below videos and more

  • DO NOT PUT OUTSIDE IN TRASH Keep in proper trash bin in hallway.
  • When you go home at your door take off items and have a trash bin to place items in.
  • Take off from CLEAN areas of the equipment. Roll off etc. Place in Trash bin and tie up.

  • Keep there and wait until the China Government or we are ready to dispose of these items and time has made sure the virus has died on them.
  • WASH HAND and Opened areas of Body 2 times with HOT Water and SOAP !!! 
  • Can use hand gel or alcohol spray
  • I advise use alcohol to spray gloves when you come in and face are, but close eyes. It will help but be safe and take great care.
  • Take HOT SHOWER WITH SOAP ! 
  • PUT Clothing directly into the washer. HOT WATER. 
  • If cannot wash with hot water then put in bag and try to place in SUN on Balcony or other.  Wait a week to wash.
  • BE SAFE AT ALL TIMES

CLSA Conference call with Coronavirus expert

Overnight in Asia, we hosted a call with professor John Nicholls a clinical professor in pathology at the University of Hong Kong and expert on coronaviruses. He was a key member of the research team at the University of Hong Kong which isolated and characterized the novel SARS coronavirus in 2003. He’s been studying coronaviruses for 25 years.

BEIJING: The death toll from China’s novel coronavirus (2019-nCoV) outbreak rose to 722 on Saturday as the country seethes over an epidemic that claimed the life of a popular doctor and created global panic.

The toll has now surpassed the number of people who died in mainland China and Hong Kong during the 2002-2003 SARS outbreak, after another 81 people succumbed to the illness in central Hubei province.

Up to 34,500 people have been infected in China by the new strain, which is believed to have emerged in a market that sold exotic animals in Hubei’s capital, Wuhan, late last year.

Scientists identify pangolin as possible coronavirus host

BEIJING: The endangered pangolin may be the link that facilitated the spread of the novel coronavirus (2019-nCoV) across China, Chinese scientists said on Friday.

At least 31,000 people have been infected and 630 killed by the virus, which has spread to two dozen countries.

Researchers at the South China Agricultural University have identified the scaly mammal as a "potential intermediate host," the university said in a statement, without providing further details.

The new virus, which emerged at a live animal market in central China's Wuhan city late last year, is believed to have originated in bats, but researchers have suggested there could have been an "intermediate host" in the transmission to humans.

Overview
In light of the threat of Ebola virus disease, it is important to emphasize the use of proper precautions for infection control in health care settings. The routes of Ebola virus transmission include direct contact with an affected person's body fluids and indirect contact by means of contaminated instruments or supplies.1 Personal protective equipment (PPE) is used when there is a risk of exposure to infectious material. PPE is designed to protect the skin and mucous membranes from exposure to pathogens.

Health care workers who use PPE to guard against contamination with the Ebola virus must remember these three principles:

• Repetitive training and demonstrated competency in putting on and removing PPE ensure proficiency in the use of the equipment.

• No skin should be exposed when wearing PPE, because of the possibility of contact with infected blood or body fluids.

• A trained observer should always be present when a health care worker is putting on or removing PPE, to identify and immediately address any breaches in protocol. The use of a checklist is recommended to document the correct sequence of steps in putting on or taking off PPE.2

This review demonstrates one procedure that will minimize the risk of exposure to infectious material when putting on and removing PPE.

She leads a group of experts to Wuhan to provide emergency assistance and develop testing kits and related technology, shortening diagnosis time.
Over the years, Chen Wei has made substantial contributions in the fight against SARS and Ebola. She is also considered as a key player in the race to develop a new coronavirus vaccine.

This interim guidance is based on what is currently known about the epidemiology of 2019-nCoV and the transmission of other viral respiratory infections. CDC will update this interim guidance as needed and as additional information becomes available.

Coronaviruses are a large family of viruses, some causing illness in people and others that circulate among animals, including camels, cats, and bats. Rarely, animal coronaviruses can infect people exposed to infected animals, and then spread among people, as has been seen with MERS-CoV and SARS-CoV, and now 2019-nCoV. This interim guidance may help prevent this virus from spreading among people in their homes and in other residential communities.

This interim guidance is intended for:

People who contracted the Wuhan virus have reported symptoms including a fever, chills, headaches, difficulty breathing, and a sore throat.

“After complaining to the local labor bureau about several things like illegal work time and not getting paid according to my contract I received my termination. When trying to call the responsible SAFEA bureau in Shijiazhuang - if they pick up at all - they hang up as soon as they realize I cannot speak Chinese. My employer has a lot of guanxi here. So also dealing with the labor bureau in Baoding is very difficult. 

I believe that my termination was unlawful based on Chinese labor law. Now that they cancelled the visa I have no idea what my legal status is. I didn't receive any certificate about the cancellation. Also no letter of release, tax documentation . And I still have the sticker for the residence permit with several months to go in my passport.

I want to stay in China and receive my outstanding salary. Could somebody explain my options to me? Many thanks for your help."

According to China Daily on January 30, currently 16 Malaysian and Bolivian tourists on the Flight OD688 from Sabah Malaysia to Tianjin were directly sent to the quarantine area in Tianjin along with other Chinese passengers on arrival. This was due to Wuhan passengers being onboard and three of them having high fevers.

In addition, the relevant departments of Dongli District are trying their best to meet the needs of the quarantined people, ensure the supply of drinking water, toiletries, daily necessities, etc., and ensure that the waste generated every day is disinfected and treated innocuously in accordance with the regulations.

It is reported that since January 29, in Tinajin, work permit applications can be approved without meeting, for a temporary time.

On January 29, 2020, Tianjin Foreign Affairs Office issued a multilingual open letter to foreigners living and working in Tianjin, providing a 24-hour service hotline, relevant prevention/work guidelines and medical information. Although no call has been received from foreigners so far, the service staff will be on duty 24 hours a day.

Give everyone a brief idea of the importance of isolation and waiting for an outbreak:

Why isolate and wait for an outbreak? Let's start by dividing the crowd into

A (infected population),

B (unknown population infected by A),

C (people suspected of being infected with A and B),

D (uninfected population) in these four categories.

Hubei province is the first to quarantine A from all groups of people, to ensure that no other people will not be infected.

But there are some As in it (about 5 million are known to be outside Wuhan and Hubei). scattered throughout the country. These people are easy to find based on information, and they've found a lot now. But A in the journey will encounter never know B, A do not know the existence of B, B also do not know the existence of A. A arrived at the destination contact C, no matter how many C contact, through A, these C can be found, has been isolated. Thus it becomes A, B, C, D four groups and the current status quo. A is isolated, C is isolated, D is stuck at home, but B does not know how much is still outside.

Now the biggest hidden danger is B, B can not find, no one knows who is B, B himself do not know whether he is B. So now everyone is isolated from each other for the purpose of sieving B with time and waiting for an outbreak of about 14 days. As soon as the outbreak occurs, B is naturally screened out. The B of this outbreak becomes A and is taken away from quarantine. Without an outbreak, it becomes a D, and you can continue to hold your ground at home. And the original isolation of C, will also appear differentiation, part of the ad become seditable, be centrally isolated, part of the d.

Then after the outbreak, there were only Two groups in society, A and D. A is centrally admitted and turned into A and returned to normal society. Without the continued treatment, some unfortunate deaths were cremated and buried. So never go out and wait for an outbreak to be a natural screening. Such as B have disease, things naturally fall out of the water. The outbreak will only end until all Bs are diseased. Therefore, all the measures taken by the State now are aimed at reducing the emergence of the new B.

Health Alert: U.S. Embassy Beijing, January 30, 2020

Location: Wuhan, Hankou area

Event: On January 29, 2020, the Department allowed for the voluntary departure of non-emergency personnel and family members of U.S. government employees.

There is an ongoing outbreak of a respiratory illness first identified in Wuhan, China, caused by a novel (new) coronavirus. In an effort to contain the novel coronavirus, the Chinese authorities have suspended air and rail travel in the area around Wuhan.

· The Department of State will be staging additional evacuation flights with capacity for private U.S. citizens on a reimbursable basis, leaving Wuhan Tianhe International Airport on or about February 3, 2020. Interested U.S. citizens in possession of valid passports should contact CoronaVirusEmergencyUSC@State.gov with the information listed below. There is no need to call to confirm receipt of your email; you will be contacted.

Chinese authorities have imposed strict travel restrictions in the area around Wuhan. Travelers should be aware that the Chinese government could prevent them from entering or exiting parts of Hubei province.

CDC: U.S. Centers for Disease Control and Prevention has issued a Warning Level 3 Alert (Avoid Nonessential Travel) for all of China due to the ongoing outbreak of respiratory illness caused by a novel (new) coronavirus that can be spread from person to person.

If you are in China, you should:

· Avoid contact with sick people.

· Avoid animals (alive or dead), animal markets, and products that come from animals (such as uncooked meat).

· Wash hands often with soap and water for at least 20 seconds. Use an alcohol-based hand sanitizer if soap and water are not available.

Continue to exercise increased caution in China due to arbitrary enforcement of local laws and special restrictions on dual U.S.-Chinese citizens.

The Chinese government has asserted broad authority to prohibit U.S. citizens from leaving China by using “exit bans,” sometimes keeping U.S. citizens in China for years. The Chinese government uses exit bans coercively:

· to compel U.S. citizens to participate in Chinese government investigations,

· to lure individuals back to China from abroad, and

· to aid Chinese authorities in resolving civil disputes in favor of Chinese parties.

In most cases, U.S. citizens only become aware of the exit ban when they attempt to depart China, and there is no method to find out how long the ban may continue. U.S. citizens under exit bans have been harassed and threatened.

U.S. citizens may be detained without access to U.S. consular services or information about their alleged crime. U.S. citizens may be subjected to prolonged interrogations and extended detention for reasons related to “state security.” Security personnel may detain and/or deport U.S. citizens for sending private electronic messages critical of the Chinese government.

Extra security measures, such as security checks and increased levels of police presence, are common in the Xinjiang Uighur and Tibet Autonomous Regions. Authorities may impose curfews and travel restrictions on short notice.

The Chinese government does not recognize dual nationality. U.S.-Chinese citizens and U.S. citizens of Chinese heritage may be subject to additional scrutiny and harassment, and the Chinese government may prevent the U.S. Embassy from providing consular services. Interim Infection Prevention and Control Recommendations for Patients with Known or Patients Under Investigation for 2019 Novel Coronavirus (2019-nCoV) in a Healthcare Setting
Background
Infection control procedures including administrative rules and engineering controls, environmental hygiene, correct work practices, and appropriate use of personal protective equipment (PPE) are all necessary to prevent infections from spreading during healthcare delivery. Prompt detection and effective triage and isolation of potentially infectious patients are essential to prevent unnecessary exposures among patients, healthcare personnel, and visitors at the facility. All healthcare facilities must ensure that their personnel are correctly trained and capable of implementing infection control procedures; individual healthcare personnel should ensure they understand and can adhere to infection control requirements.

This guidance is based on the currently limited information available about 2019-nCoV related to disease severity, transmission efficiency, and shedding duration. This cautious approach will be refined and updated as more information becomes available and as response needs change in the United States. This guidance is applicable to all U.S. healthcare settings. This guidance is not intended for non-healthcare settings (e.g., schools) OR to persons outside of healthcare settings. For recommendations regarding clinical management, air or ground medical transport, or laboratory settings, refer to the main CDC 2019-nCoV website.

Definition of Healthcare Personnel (HCP) – For the purposes of this guidance, HCP refers to all persons, paid and unpaid, working in healthcare settings engaged in patient care activities, including: patient assessment for triage, entering examination rooms or patient rooms to provide care or clean and disinfect the environment, obtaining clinical specimens, handling soiled medical supplies or equipment, and coming in contact with potentially contaminated environmental surfaces.

Recommendations
1. Minimize Chance for Exposures
Ensure facility policies and practices are in place to minimize exposures to respiratory pathogens including 2019-nCoV. Measures should be implemented before patient arrival, upon arrival, and throughout the duration of the affected patient’s presence in the healthcare setting.

Before ArrivalWhen scheduling appointments, instruct patients and persons who accompany them to call ahead or inform HCP upon arrival if they have symptoms of any respiratory infection (e.g., cough, runny nose, fever1) and to take appropriate preventive actions (e.g., wear a facemask upon entry to contain cough, follow triage procedures).
If a patient is arriving via transport by emergency medical services (EMS), the driver should contact the receiving emergency department (ED) or healthcare facility and follow previously agreed upon local or regional transport protocols. This will allow the healthcare facility to prepare for receipt of the patient.
 

Upon Arrival and During the VisitTake steps to ensure all persons with symptoms of suspected 2019-nCoV or other respiratory infection (e.g., fever, cough) adhere to respiratory hygiene and cough etiquette, hand hygiene, and triage procedures throughout the duration of the visit. Consider posting visual alerts (e.g., signs, posters) at the entrance and in strategic places (e.g., waiting areas, elevators, cafeterias) to provide patients and HCP with instructions (in appropriate languages) about hand hygiene, respiratory hygiene, and cough etiquette. Instructions should include how to use facemasks (See definition of facemask in Appendix) or tissues to cover nose and mouth when coughing or sneezing, to dispose of tissues and contaminated items in waste receptacles, and how and when to perform hand hygiene.
Ensure that patients with symptoms of suspected 2019-nCoV or other respiratory infection (e.g., fever, cough) are not allowed to wait among other patients seeking care.  Identify a separate, well-ventilated space that allows waiting patients to be separated by 6 or more feet, with easy access to respiratory hygiene supplies. In some settings, patients might opt to wait in a personal vehicle or outside the healthcare facility where they can be contacted by mobile phone when it is their turn to be evaluated.
Ensure rapid triage and isolation of patients with symptoms of suspected 2019-nCoV or other respiratory infection (e.g., fever, cough):Identify patients at risk for having 2019-nCoV infection before or immediately upon arrival to the healthcare facility.
Implement triage procedures to detect patients under investigation (PUI) for 2019-nCoV during or before patient triage or registration (e.g., at the time of patient check-in) and ensure that all patients are asked about the presence of symptoms of a respiratory infection and history of travel to areas experiencing transmission of 2019-nCoV or contact with possible 2019-nCoV patients.
Implement respiratory hygiene and cough etiquette (i.e., placing a facemask over the patient’s nose and mouth if that has not already been done) and isolate the PUI for 2019-nCoV in an Airborne Infection Isolation Room (AIIR), if available. See recommendations for “Patient Placement” below. Additional guidance for evaluating patients in U.S. for 2019-nCoV infection can be found on the CDC 2019-nCoV website.
Inform infection prevention and control services, local and state public health authorities, and other healthcare facility staff as appropriate about the presence of a patient under investigation for 2019-nCoV.
Provide supplies for respiratory hygiene and cough etiquette, including 60%-95% alcohol-based hand rub (ABHR), tissues, no touch receptacles for disposal, and facemasks at healthcare facility entrances, waiting rooms, patient check-ins, etc.
2. Adherence to Standard, Contact and Airborne Precautions, Including the Use of Eye Protection
Standard Precautions assume that every person is potentially infected or colonized with a pathogen that could be transmitted in the healthcare setting. Elements of Standard Precautions that apply to patients with respiratory infections, including those caused by 2019-nCoV, are summarized below. Attention should be paid to training and proper donning (putting on), doffing (taking off), and disposal of any PPE. This document does not emphasize all aspects of Standard Precautions (e.g., injection safety) that are required for all patient care; the full description is provided in the Guideline for Isolation Precautions: Preventing Transmission of Infectious Agents in Healthcare Settings. All HCP (see section 3 for measures for non-HCP visitors) who enter the room of a patient with suspected or confirmed 2019-nCoV should adhere to Standard, Contact, and Airborne Precautions, including the following:

Patient PlacementPlace a patient with known or suspected 2019-nCoV (i.e., PUI) in an AIIR that has been constructed and maintained in accordance with current guidelines.AIIRs are single patient rooms at negative pressure relative to the surrounding areas, and with a minimum of 6 air changes per hour (12 air changes per hour are recommended for new construction or renovation). Air from these rooms should be exhausted directly to the outside or be filtered through a high-efficiency particulate air (HEPA) filter before recirculation. Room doors should be kept closed except when entering or leaving the room, and entry and exit should be minimized. Facilities should monitor and document the proper negative-pressure function of these rooms.
If an AIIR is not available, the patient should be transferred as soon as is feasible to a facility where an AIIR is available or discharged to home (in consultation with state or local public health authorities) if deemed medically appropriate. Pending transfer, place a facemask on the patient and isolate him/her in an examination room with the door closed. The patient should not be placed in any room where room exhaust is recirculated within the building without HEPA filtration.
Once in an AIIR, the patient’s facemask may be removed. Limit transport and movement of the patient outside of the AIIR to medically-essential purposes. When not in an AIIR (e.g., during transport or if an AIIR is not available), patients should wear a facemask to contain secretions.
Personnel entering the room should use PPE, including respiratory protection, as described below.
Only essential personnel should enter the AIIR. Implement staffing policies to minimize the number of HCP who enter the room.Facilities should consider caring for these patients with dedicated HCP to minimize risk of transmission and exposure to other patients and other HCP.
Facilities should keep a log of all persons who care for or enter the rooms or care area of these patients.
Use dedicated or disposable noncritical patient-care equipment (e.g., blood pressure cuffs). If equipment will be used for more than one patient, clean and disinfect such equipment before use on another patient according to manufacturer’s instructions.
HCP entering the AIIR soon after a patient vacates the room should use respiratory protection. (See personal protective equipment section below) Standard practice for pathogens spread by the airborne route (e.g., measles, tuberculosis) is to restrict unprotected individuals, including HCP, from entering a vacated room until sufficient time has elapsed for enough air changes to remove potentially infectious particles (more information on clearance rates under differing ventilation conditions is available). We do not yet know how long 2019-nCoV remains infectious in the air.  In the interim, it is reasonable to apply a similar time period before entering the room without respiratory protection as used for pathogens spread by the airborne route (e.g., measles, tuberculosis). In addition, the room should undergo appropriate cleaning and surface disinfection before it is returned to routine use.
Hand HygieneHCP should perform hand hygiene using ABHR before and after all patient contact, contact with potentially infectious material, and before putting on and upon removal of PPE, including gloves. Hand hygiene in healthcare settings also can be performed by washing with soap and water for at least 20 seconds. If hands are visibly soiled, use soap and water before returning to ABHR.
Healthcare facilities should ensure that hand hygiene supplies are readily available to all personnel in every care location.
Personal Protective Equipment
Employers should select appropriate PPE and provide it to HCP in accordance with OSHA’s PPE standards (29 CFR 1910 Subpart I)external icon. HCP must receive training on and demonstrate an understanding of when to use PPE; what PPE is necessary; how to properly don, use, and doff PPE in a manner to prevent self-contamination; how to properly dispose of or disinfect and maintain PPE; and the limitations of PPE. Any reusable PPE must be properly cleaned, decontaminated, and maintained after and between uses. Facilities should have policies and procedures describing a recommended sequence for safely donning and doffing PPE:GlovesPerform hand hygiene, then put on clean, non-sterile gloves upon entry into the patient room or care area. Change gloves if they become torn or heavily contaminated.
Remove and discard gloves when leaving the patient room or care area, and immediately perform hand hygiene.
GownsPut on a clean disposable gown upon entry into the patient room or area. Change the gown if it becomes soiled. Remove and discard the gown before leaving the patient room or care area.
Respiratory ProtectionUse respiratory protection (i.e., a respirator) that is at least as protective as a fit-tested NIOSH-certified disposable N95 filtering facepiece respirator before entry into the patient room or care area. See appendix for respirator definition.
Disposable respirators should be removed and discarded after exiting the patient’s room or care area and closing the door. Perform hand hygiene after discarding the respirator.
If reusable respirators (e.g., powered air purifying respirator/PAPR) are used, they must be cleaned and disinfected according to manufacturer’s reprocessing instructions prior to re-use.
Respirator use must be in the context of a complete respiratory protection program in accordance with Occupational Safety and Health Administration (OSHA) Respiratory Protection standard (29 CFR 1910.134external icon). Staff should be medically cleared and fit-tested if using respirators with tight-fitting facepieces (e.g., a NIOSH-certified disposable N95) and trained in the proper use of respirators, safe removal and disposal, and medical contraindications to respirator use.
Eye ProtectionPut on eye protection (e.g., goggles, a disposable face shield that covers the front and sides of the face) upon entry to the patient room or care area. Remove eye protection before leaving the patient room or care area. Reusable eye protection (e.g., goggles) must be cleaned and disinfected according to manufacturer’s reprocessing instructions prior to re-use.  Disposable eye protection should be discarded after use.
Use Caution When Performing Aerosol-Generating ProceduresSome procedures performed on 2019-nCoV patients could generate infectious aerosols. In particular, procedures that are likely to induce coughing; e.g., nasopharyngeal specimen collection, sputum induction, and open suctioning of airways should be performed cautiously and avoided if possible.
If performed, these procedures should take place in an AIIR, and personnel should use respiratory protection as described above. In addition:Limit the number of HCP present during the procedure to only those essential for patient care and procedural support.
Clean and disinfect procedure room surfaces promptly as described in the section on environmental infection control below.
Duration of Isolation PrecautionsUntil information is available regarding viral shedding after clinical improvement, discontinuation of isolation precautions should be determined on a case-by-case basis, in conjunction with local, state, and federal health authorities.
Factors that should be considered include: presence of symptoms related to 2019-nCoV, date symptoms resolved, other conditions that would require specific precautions (e.g., tuberculosis, Clostridioides difficile), other laboratory information reflecting clinical status, alternatives to inpatient isolation, such as the possibility of safe recovery at home.
3. Manage Visitor Access and Movement Within the Facility
Establish procedures for monitoring, managing and training visitors.
Restrict visitors from entering the room of known or suspected 2019-nCoV patients (i.e., PUI). Alternative mechanisms for patient and visitor interactions, such as video-call applications on cell phones or tablets should be explored. Facilities can consider exceptions based on end-of-life situations or when a visitor is essential for the patient’s emotional well-being and care.
Visitors to known or suspected 2019-nCoV (i.e., PUI) patients should be scheduled and controlled to allow for:Screening visitors for symptoms of acute respiratory illness before entering the healthcare facility.
Facilities should evaluate risk to the health of the visitor (e.g., visitor might have underlying illness putting them at higher risk for 2019-nCoV) and ability to comply with precautions.
Facilities should provide instruction, before visitors enter patients’ rooms, on hand hygiene, limiting surfaces touched, and use of PPE according to current facility policy while in the patient’s room.
Facilities should maintain a record (e.g., log book) of all visitors who enter patient rooms.
Visitors should not be present during aerosol-generating procedures.
Visitors should be instructed to limit their movement within the facility.
Exposed visitors (e.g., contact with symptomatic 2019-nCoV patient prior to admission) should be advised to report any signs and symptoms of acute illness to their health care provider for a period of at least 14 days after the last known exposure to the sick patient.
All visitors should follow respiratory hygiene and cough etiquette precautions while in the common areas of the facility.
4. Implement Engineering Controls
Consider designing and installing engineering controls to reduce or eliminate exposures by shielding HCP and other patients from infected individuals. Examples of engineering controls include physical barriers or partitions to guide patients through triage areas, curtains between patients in shared areas, closed suctioning systems for airway suctioning for intubated patients, as well as appropriate air-handling systems (with appropriate directionality, filtration, exchange rate, etc.) that are installed and properly maintained.
5. Monitor and Manage Ill and Exposed Healthcare Personnel
Movement and monitoring decisions for HCP with exposure to 2019-nCoV should be made in consultation with public health authorities.
Facilities and organizations providing healthcare should implement sick leave policies for HCP that are non-punitive, flexible, and consistent with public health guidance.
 6. Train and Educate Healthcare Personnel
Provide HCP with job- or task-specific education and training on preventing transmission of infectious agents, including refresher training.
HCP must be medically cleared, trained, and fit tested for respiratory protection device use (e.g., N95 filtering facepiece respirators), or medically cleared and trained in the use of an alternative respiratory protection device (e.g., Powered Air-Purifying Respirator, PAPR) whenever respirators are required. Ensure that HCP are educated, trained, and have practiced the appropriate use of PPE prior to caring for a patient, including attention to correct use of PPE and prevention of contamination of clothing, skin, and environment during the process of removing such equipment.
7. Implement Environmental Infection Control
Dedicated medical equipment should be used for patient care.
All non-dedicated, non-disposable medical equipment used for patient care should be cleaned and disinfected according to manufacturer’s instructions and facility policies.
Ensure that environmental cleaning and disinfection procedures are followed consistently and correctly.
Routine cleaning and disinfection procedures (e.g., using cleaners and water to pre-clean surfaces prior to applying an EPA-registered, hospital-grade disinfectant to frequently touched surfaces or objects for appropriate contact times as indicated on the product’s label) are appropriate for 2019-nCoV in healthcare settings, including those patient-care areas in which aerosol-generating procedures are performed. Products with EPA-approved emerging viral pathogens claims are recommended for use against 2019-nCoV. These products can be identified by the following claim:“[Product name] has demonstrated effectiveness against viruses similar to 2019-nCoV on hard non-porous surfaces. Therefore, this product can be used against 2019-nCoV when used in accordance with the directions for use against [name of supporting virus] on hard, non-porous surfaces.”
This claim or a similar claim, will be made only through the following communications outlets: technical literature distributed exclusively to health care facilities, physicians, nurses and public health officials, “1-800” consumer information services, social media sites and company websites (non-label related). Specific claims for “2019-nCoV” will not appear on the product or master label.

If there are no available EPA-registered products that have an approved emerging viral pathogen claim for 2019-nCoV, products with label claims against human coronaviruses should be used according to label instructions.
Management of laundry, food service utensils, and medical waste should also be performed in accordance with routine procedures.

8. Establish Reporting within Healthcare Facilities and to Public Health Authorities
Implement mechanisms and policies that promptly alert key facility staff including infection control, healthcare epidemiology, facility leadership, occupational health, clinical laboratory, and frontline staff about known or suspected 2019-nCoV patients (i.e., PUI).
Communicate and collaborate with public health authorities.Promptly notify state or local public health authorities of known or suspected 2019-nCoV patients (i.e., PUI). Facilities should designate specific persons within the healthcare facility who are responsible for communication with public health officials and dissemination of information to HCP.

Information about Respirators:

A respirator is a personal protective device that is worn on the face, covers at least the nose and mouth, and is used to reduce the wearer’s risk of inhaling hazardous airborne particles (including dust particles and infectious agents), gases, or vapors. Respirators are certified by the CDC/NIOSH, including those intended for use in healthcare.
Respirator use must be in the context of a complete respiratory protection program in accordance with OSHA Respiratory Protection standard (29 CFR 1910.134external icon). Filtering Facepiece Respirators (FFR) including N95 Respirators

A commonly used respirator is a filtering facepiece respirator (commonly referred to as an N95). Filtering facepiece respirators are disposable half facepiece respirators that filter out particles.
To work properly, FFRs must be worn throughout the period of exposure and be specially fitted for each person who wears one (this is called “fit-testing” and is usually done in a workplace where respirators are used).

Powered Air-Purifying Respirators (PAPRs)

Powered air-purifying respirators (PAPRs) have a battery-powered blower that pulls air through attached filters, canisters, or cartridges. They provide protection against gases, vapors, or particles, when equipped with the appropriate cartridge, canister, or filter.
Loose-fitting PAPRs do not require fit testing and can be used with facial hair.

If worn properly, a facemask helps block respiratory secretions produced by the wearer from contaminating other persons and surfaces (often called source control).

Doctor's advice.

About the virus:

1. It is pretty large in size (cell is about 400-500nm diameter), so any normal mask (not just the N95 feature) should be able to filter it out. However, when someone who's infected sneezes in front of you, it will take a great 3 meters (about 10 feet) before it drops to the ground and is no longer airborne.

2. When the virus drops on metal surface, it will live for at least 12 hours. So remember if you come in contact with any metal surface, wash your hands with soap thoroughly.

3. The virus can remain active on fabric for 6-12 hours. Normal laundry detergent should kill the virus. For winter clothing that does not require daily washing, you can put it out under the sun to kill the virus.

About the symptoms of the pneumonia caused by Coronavirus:

1. It will first infect the throat, so the throat will have the dry sore throat feeling which will last for 3 to 4 days

2. Then the virus will blend into the nasal fluid and drips into the trachea and enter the lungs, causing pneumonia. This process will take 5 to 6 days.

3. With pneumonia, comes high fever and difficulty in breathing. The nasal congestion is not like the normal kind. You will feel like you are drowning in water. It's important to go seek immediate medical attention if you feel like this.

About prevention:

1. The most common way of getting infected is by touching things in public, so you must wash your hands frequently. The virus can only live on your hands for 5-10 mins, but a lot can happen in those 5-10 mins (you can rub your eyes or pick your nose unwittingly).

2. Aside from washing your hands frequently, you can gargle with Betadine Sore Throat Gargle to eliminate or minimize the germs while they are still in your throat (before dripping down to your lungs).
